Here’s how we do it:

1. Drastically Reduce Readmission Rates for Diabetes, Hypertension, and CHF.

The national 30-day readmission rate for these conditions is between 18-25%, costing patients anywhere from $11,000-$15,000 for a 3-day stay.

Sequoia has reduced the readmission rate to less than 1% for over 1,600 patients and counting. 

2. We Start With House Call Services, But We Don’t Stop There.

Qualified patients will get a visit from a health care provider in their own home. A full physical exam is done with each visit, and a comprehensive plan of care is created and explained to the patient.

3. Telemedicine Tools Continue Care Long After the Provider Leaves the Home.

Qualified patients will get monthly telemedicine visits via computer to review their plan of care, adjust their medical plans of care, and request prescription refills. Any adjustments to medications will be consulted and provided during this visit.

4. Providing 24/7 Patient Monitoring With Free Devices and On-Call Providers.

Qualified patients will receive a free device to measure their blood pressure, weight, and blood sugars. Each patient will be assigned to an RN who will be monitoring these numbers 24/7, and can be accessed anytime of the day or night. 

Additionally, this RN helps manage the patient’s chronic care needs, via updates regarding their plans of care, and oversight on their outcomes with ancillary services like physical therapy, wound care etc.

What areas do you cover for service?

We cover Houston and all greater Houston area, from Conroe to Lake Jackson and Katy to Bay town.

How quickly can the patient be seen?

Patient will be seen by their primary care provider within 72 hours for an in home visit or 24 hours for a telemedicine appointment once insurance approval has been obtained

Do you provide an on-call number?

Yes, each patient who is receiving primary care services through Sequoia will be given an on-call number to contact in case of emergency.

Do you provide dietary education for disease management?

Yes, the healthcare providers will educate you on the best approach to diet and exercise and customize the plan to each specific disease process.
