Telemedicine
Qualified patients will get monthly telemedicine visits to review their plan of care, medical plans of care will be adjusted, prescription refills or any adjustments to medications will be provided during this visit.
House call services
Qualified patients will get a visit from a health care provider in their own home, a full physical exam is done with each visit as well as a comprehensive plan of care.
Remote patient monitoring with 24/7 access to an on-call provider
Qualified patients who have Hypertension, Congestive Heart Failure or Diabetes will receive a device for free to measure their blood pressure, weight as well as blood sugars. The patient will be assigned to a RN who will be monitoring these numbers 24/7. The patient’s will have 24/7 access to a provider.
Chronic Care Management
Qualified patients will be assigned to a RN who will be helping manage the patient’s chronic conditions. The patients will be receiving updates regarding their plans of care, oversight on their outcomes with ancillary services such as physical therapy, wound care etc.
